關於計劃 ABOUT THE PROJECT

「賽馬會紓緩童行計劃」 由香港賽馬會慈善信託基金捐助,兒童癌病基金策動,以「孩子為中心,家庭為本」為使命,開展一項為期三年的兒童紓緩服務計劃,此計劃標誌著基金以優質服務為患有危重症病童及其家庭提供全面支持邁出新的里程碑。 

 

本計劃目標將兒童紓緩服務擴展至兒童深切治療部和新生嬰兒深切治療部,為患有危重症病童及其家庭於身心社靈及生活上給予多方支持,並循證實踐,建立適合本地的兒童紓緩療護模式。 

跨專業團隊的服務期望為 960名患有危重症病童及其家人提供優質及全面的關顧。同時亦致力進行培訓工作,目標培訓超過 1,700名醫護人員和照顧者,讓他們掌握相關技能及知識,以提升服務的質量。 

 

透過「賽馬會紓緩童行計劃」,我們會以公眾教育,讓大眾更了解兒童紓緩服務;並在社區傳揚尊重和珍愛生命的信念,期望不但可為患有危重症病童及其家庭給予全面及適切的支援,也同時提升人民的精神健康。 

 

The Children’s Cancer Foundation (CCF) is proud to introduce “Jockey Club Children’s Palliative Care Project”. This is a three-year project generously supported by the Hong Kong Jockey Club Charities Trust (JCCT), is a significant stride by CCF towards providing holistic support to children and their families facing life-threatening conditions.   

The project allows us to extend our unique children’s palliative care to PICU and NICU patients and their families. This community care model encompasses clinical, social, psychological, spiritual, and practical support domains, which keen on establishing a sustainable and evidence-based practice.

Our professional and multidisciplinary teams are poised to deliver a high-quality “child-centered and family-oriented” care to up to 960 high-impact beneficiaries, including children, teenagers, and their families. We are also committed to train over 1700 health service providers and carers and equip them with the relevant skills and knowledge on their caring journey.    

Through this project, we strive to bring hope to our targeted patients and families, letting them feel the care and support from the wider community and facilitate them to live life to the fullest.

為兒科病人提供紓緩治療和晚期照顧,以提升病人的生理、心理和社交健康

To provide palliative care and end-of-life care for paediatric patients to improve their bio-psycho-social wellbeing 

通過加強照顧者/父母的護理技能和改善他們的心理社交健康以此賦能照顧者

To empower carers/parents through enhancing their caregiving skills and improving their psychosocial wellbeing 

建立社區支持能力和推廣公眾對兒童紓緩服務的認知

To build up community capacity and to promote awareness of CPC services

  1. 活動費用全免。

  2. 截止報名日期為 2024年4月18日(四),席位有限,先到先得。

  3. 報名成功後將會透過電子郵件確認。如於2024年4月19日(五)前未收到確認信,請與大會聯繫。

  4. 海外與會人士如需要報名文件用於申請簽證事宜,請與大會聯繫。

  5. 活動結束後,將通過電子郵件向參會者頒發出席證書。

  6. 與會人士務必確認出席,並填寫活動意見問卷。大會將遞交有關記錄至相關學會,以進行CME/CNE/CPD認證。

  7. 活動期間拍攝的照片和影片或會作日後推廣及宣傳之用途。

Registration Notice

  1. The event is free of charge.

  2. The deadline for registration is April 18th, 2024 (Thu). Space is limited for both onsite and online participants on a first-come-first-served basis.

  3. Successful participants will be confirmed by email before April 19th, 2024 (Fri). Please contact the Organising Committee for any enquiries. 

  4. For overseas participants, please contact the Organising Committee if supporting documents are needed to apply for a travel visa.

  5. e-Certificates of Attendance will be issued to the participants after the completion of the event feedback form.

  6. To facilitate accreditation, participants should attend and complete the event feedback form. Both physical and virtual attendance lists will be sent to the related academies to grant accreditation.

  7. Photos and videos taken during the event may used for promotional or publicity purposes.
